Food & Beverage Request Form

This form must be completed for all on-campus events for review. Completion of form is not a guarantee of approval. The form is available to students, departments or non-USC clients intending to provide food service at an event. Please direct any questions to hosp.approvals@usc.edu.

You do not need to complete this form if you have contracted with USC Hospitality Private Events & Conferences.

1 Guidelines & Requirements

You MUST contract with a University approved caterer that has current forms required by University Procurement*:

  • City Health Permit
  • General Liability Insurance (with USC listed as an additional insured)
    • This Certificate of Insurance MUST have USC listed as an additional insured.
    • The vendor can use the following name and address when updating their certificate:

  • Auto Liability Insurance
    • This Certificate of Insurance is not needed if you are picking up food at the food vendors location and it isn’t being delivered to campus.

* You will be prompted to upload any documents that aren’t current with University Procurement

Documentation (health permits, insurance, etc.) are not required for pre-made or packaged items from national grocery chains like Costco, Smart & Final, Vons, Ralphs, Trader Joes, etc.

No homemade items of any type are allowed at your event.

All non-alcoholic beverages must be Coke branded, including the water.

To be in line with the University's Sustainability Initiatives, no PLASTIC beverage bottles/containers are to be offered at any events.

Food and beverage items may not be sold at an event. All fundraising events must be approved through the fundraising approval process (fundraising form).

BBQ events with more than 35 guests require a University approved caterer.

Any events serving alcohol require separate approval.

Approval forms must be submitted two weeks prior to the event date. Failure to comply may result in the removal of unauthorized food service during your event.

Please note: you must first complete the University Events Permit Application (UEPA) prior to completing this form. Depending on when you completed the UEPA, you should have received an ID in a format like "12345" or "A-00012345." Please begin this form by entering that ID in the Event ID Number field below.

2 Event Details

You are encouraged to utilize the services of USC Hospitality Private Events & Conferences (private.events@usc.edu) or University Club Delivery (uclubdelivery@usc.edu) for your event. Doing so allows funds to stay within the Trojan Family and is in support of student programs. USC Departments may enjoy additional savings (tax exempt status) with qualifying events. Request a proposal from University Club Delivery at https://hospitality.usc.edu/submit-an-rfp/.

Note: All fields are required unless otherwise indicated. If your event has more than 10 caterers please submit multiple requests and indicate the total number of caterers for the event on each individual request for "Number of Caterers/Vendors."

3 Contact Info

Food Truck Info

USC Departments, Schools and Institutes can only bring food trucks on campus in the capacity of catering for an event. They must be contracted as you would any other caterer: select a menu, negotiate pricing for a specific number of guests and arrange payment. Cash sales from a food truck are not allowed. If you are bringing in a food truck, the truck MUST be an approved vendor (documentation like any other caterer), have a fire extinguisher on board, protect the ground from oil/grease spills and if approved by FMS to have tires on the grass, must place plywood under the tires. You also have to pay for an oversized parking permit (check with Transportation). The location of the food truck must also be approved by DPS and Fire Safety in advance. This includes the arrival/departure path for the truck. Pedestrian safety must be taken into account and planned for accordingly.


University recognized student organizations are allowed (with prior approval) to have food trucks on campus for fundraising efforts. Food trucks for this purpose can only be stationed in McCarthy Quad (concrete inlet on the southern in towards Doheny Library) , on Trousdale adjacent to VKC and Pardee Plaza (between Bloom Walk and Exposition Blvd).
