Alcohol Request Form

Your organization / department must complete this form with all necessary approvals and documentation at least two (2) weeks prior to your event. A copy must be available for inspection at all times at the site of the event. Completion of this form does not guarantee approval. For university recognized student organizations, Campus Activities will contact you for final approval.

1 Alcohol Service Guidelines & Requirements

Alcohol only events will NOT be approved. A variety of food items must be at all events that serve alcohol.

All alcohol must be served by a University approved vendor (approved vendors listed in drop down options below). One-time caterer approvals are not applicable to alcohol, you must use a vendor that has completed the Procurement Services application.

Self-service bars (either by guests or department staff) are NEVER approved.

When submitting your alcohol request, you will need to upload a completed Alcohol Request Form and any procurement forms that are not current*. These vendor forms include:

  • ABC License
  • General Liability Insurance
  • Liquor Liability Insurance
  • Auto Liability Insurance
  • Workers Compensation Insurance

Students who are of legal drinking age may not be served more than two alcoholic beverages at recognized university student organization events. Additionally, all guests who are verified as 21 and over, must be provided with a two-tab wristband. The wristbands must be distributed by trained bar staff or CSE/DPS at check-in.

USC Hospitality is the ONLY vendor allowed to serve at approved Beer Gardens on campus.

Approval forms must be submitted two weeks prior to the event date. Failure to comply may result in the removal of unauthorized food service during your event.

Submission of this request form indicates acknowledgement of, and agreement to, these Alcohol Service Guidelines & Requirements.

* You will be prompted to upload any documents that aren’t current with University Procurement.

Please note: you must first complete the University Events Permit Application (UEPA) prior to completing this form. Depending on when you completed the UEPA, you should have received an ID in a format like "12345" or "A-00012345." Please begin this form by entering that ID in the Event ID Number field below.
